FOOTBALL PRACTICE IS TUESDAY-THURSDAY
13U & 9U 6-8 PM

Football Practice is at Hormel Stadium unless there is a Medford High School home game

New players need to wear cleats, and bring a water bottle and a mouth guard that attaches to the helmet.
Parents please bring any remaining player paperwork to practice.

Please note the Participation Contract and Medical History forms must be filled out and uploaded as part of the registration process (a scan is best). We will need to collect the signed paper copies at practice. Please see the section below for all the documents Pop Warner requires us to collect.

THE FOLLOWING DOCUMENTS ARE NEEDED FOR PARTICIPATION:
Proof of Age
Proof of age can be either a child's Birth Certificate with raised seal or theiir Passport.
This is required for all NEW players & cheerleaders. Participants from last year do not need to provide this again.
2024-2025 PARTICIPANT CONTRACT AND PARENTAL CONSENT FORM
This form must be uploaded at registration. Participant's name must match birth certificate exactly.
Final Report Card 2023-2024
Last report card of the 2023-2024 school year. Please bring a complete paper copy to the first practice.
2024 Medical History Form
PHYSICALS ARE NO LONGER REQUIRED, BUT... this form must be filled out COMPLETELY by parent or guardian, and uploaded at registration.
2024 Football Age Groups:
(Please note that, with the exception of 6U Rookie, Pop Warner requires a minimum of 16 registered players in an age group to field a team)
A child’s age on July 31, 2024 is his/her age for the season.
6U (Rookie 7-on-7) ages 5-6
9U ages 7-8-9
13U ages 11-12-13
(we will not have 7U or 11U this year)

FINANCIAL AID WILL BE AVAILABLE TO ANYONE WHO NEEDS IT
