Please note that you no longer need to upload any documents during on-line registration!  The buttons to upload documents are still in the registration, but they are not required!
ONLY FIELDS WITH AN * ASTERISK ARE REQUIRED
You will “e-sign” the Medical History Form and Participation Contract during registration!

We will be hosting some in-person registrations sessions we will post soon.  In these sessions we will walk you through the registration process.  Also, if you would like to pay with cash or check, we can also do that at these sessions.

We will still need to physically collect “Proof of Age” as a birth certificate or passport (for new players), and get final report cards for all players once they are available at the end of the school year! 

For Report Cards, please EMAIL us a .pdf from School Brains once the final report cards are available, or bring a complete paper copy to an in-person registration/document collection or the first practice.
